📋 Methodology & Selection Criteria

Every entity in this report was identified through statistical anomaly detection on billing data alone. No demographic, geographic, or name-based criteria were used in selection. The names, owners, and addresses of flagged entities were retrieved after identification — not before.

How we found them:

  1. Queried CMS public use files and Illinois Medicaid provider data for all clinical lab providers (taxonomy 291U00000X) with first billing date after January 2020
  2. Flagged entities with anomalous billing trajectories: >$1M total billing, hockey-stick growth patterns, or billing concentrated in a single HCPCS code representing >80% of revenue
  3. Identified billing code migration patterns — entities that stopped billing one high-reimbursement code and immediately began billing another at comparable volume
  4. Cross-referenced flagged NPIs against the OIG LEIE exclusion database and Illinois HFS sanctions list (zero matches)
  5. Retrieved physical addresses from NPPES registry and verified via Google Street View imagery
  6. Retrieved authorized official names from the CMS NPPES API — these are the individuals who signed the Medicare/Medicaid enrollment applications

1. Suspend and audit every flagged NPI

41 labs. Every NPI is listed in this report. CMS can suspend billing pending investigation — they just haven't. Each entity should demonstrate it has the physical capacity, equipment, and staff to perform the volume of tests it billed for.

2. Reform NPI registration

You can register an NPI and start billing Medicaid in days. Four owners in this report have "First Name Unknown" in federal records. Require verified identity, site inspections, and capacity documentation before any new lab can bill.

3. Flag billing code pivots automatically

When a lab stops billing one code and immediately starts billing another at the same volume, that should trigger automatic review. The relay race pattern — COVID → 87913 → genomics → drug testing — is detectable in real time. Nobody is looking.

4. Investigate the networks
